Position Opening

ACIL Community Living Services Program (CLSP) Coordinator/Transportation Coordinator

(Full-Time Non-Exempt Position)

 

ESSENTIAL JOB FUNCTIONS:

 

  1. Responsible for the implementation of ACIL’s CLSP Program in our defined service area in which is currently the counties of Boone, Braxton, Calhoun, Clay, Doddridge, Greenbrier, Jackson, Kanawha, Nicholas, Pleasants, Putnam, Ritchie, Roane, Webster, Wirt, and Wood. 
  2. Monitor the Community Living Services Program waiting list and purchases of home modifications, wheelchair ramps, vehicle modifications, assistive devices, and durable medical equipment. Based on the appropriate funding sources.
  3. Knowledge of ADA Standards for accessible design. 
  4. Must complete intakes, needed documentation, and reports.
  5. Answer phones in a professional manner.
  6. Promote the Independent Living Philosophy in the community.
  7. Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections. 
  8. Maintain effective and positive working relationships with all disability partners as well as other agencies/service providers to encourage referrals, promote cooperation, and ensure effectiveness of services.
  9. Ability to work with diverse consumers on a cross disabilities basis and understand their independent living needs to find resources in the community for referrals.
  10. Prepare and submit reports as required in accordance with ACIL’s Operational Policies and Procedures.
  11. Identifies third party resources, identifies vendors, subcontractors, and obtains bids in accordance with ACIL’s policies and procedures.
  12. Provides CLSP documentation for case service records (example: bids, pictures, copies of checks, etc.).
  13. Conducts assessments and writes reports to determine what modifications needs to be made and determines specifications for wheelchair ramps.
  14. Make referrals to other programs and locates alternative funding sources.
  15. Explores options for purchasing multiple items and/or services at a discounted rate. 
  16. Schedule and provide safe and accessible nonemergent transportation (NEMT) as requested.
  17. Coordinate consumer transportation services with Center staff, consumers, Modivcare, and UMWA.
  18. Maintain, clean, sanitize vehicles and adhere to maintenance schedule.
  19. Attend meetings in the community that relates to transportation and reports to Center staff on new programs and developments.
  20. Submit billing information to the Office Manager for billing.
  21. Adhere to all transportation contract requirements and other required documentation.
  22. Attend meetings and trainings related to this position.
  23. Maintain and clean office as needed.
  24. Must document consumer information and services using CIL Management Suites. Must document regularly.
  25. Regular attendance and punctuality is required.
  26. Provide services in compliance with HIPPA (confidentiality) requirements.
  27. Perform related duties as assigned.

 

 

MINIMUM JOB QUALIFICATIONS:

Education:

Associate Degree in Human Services, Social Services, Rehabilitation Services, or closely related field. Knowledge of the ADA Standards for accessible design.

 

High school diploma and a contractor’s license or construction background may be substituted. 

 

Desirable Experience/Knowledge:

•                   Personal experience with a disability or working with persons with disabilities is preferred

•                   Ability to interact with consumers as a peer.

•                   Knowledge of the Americans with Disabilities Act (ADA) and ADA standards for accessible design.

•                   Knowledge in construction and building.

 

Skills, Knowledge and Abilities:  

•                   Must be a self-starter with the ability to organize tasks and manage time effectively.

•                   Must have excellent interactive skills and ability to motivate others.

•                   Requires excellent written and communication skills.

•                   Working knowledge of Microsoft Office, internet, and email.

•                   Ability to work with management and staff to achieve goals of the organization.

•                   Ability to work independently, one-on-one or in a group setting to provide support with the ongoing daily operations of a non-profit organization.

•                   Must be able to travel throughout the sixteen-county service area independently. May occasionally attend meetings, trainings, and conferences during non-working hours.

•                   Must have driver’s license and good driving record.

 


Salary Range:

Salary is negotiable, plus benefits including dental insurance, vision insurance, disability insurance, IRA with company match, annual, holiday, and sick leave.
